  # Why Full-Text Access Matters in Biomedical Research

MEDLINE helps researchers discover authoritative biomedical literature, but full-text access enables them to evaluate methods, findings and limitations and apply evidence more effectively.

**What You'll Learn:**

- Why **full text is an important complement** to MEDLINE indexing
- How full-text access to biomedical literature **supports clinical care, research and medical education**
- Why **international coverage, Ahead-of-Print content and embargoes matter**
- How libraries can **connect PubMed discovery** with available full text

For medical libraries, helping users discover relevant biomedical literature is only part of the research process. Clinicians, researchers, faculty and students ultimately need access to the evidence itself to evaluate it and determine how it applies to their questions.

For decades, MEDLINE has provided an authoritative foundation for biomedical literature discovery. Its rigorous indexing and MeSH Subject Headings help researchers identify relevant literature across medicine, nursing, dentistry, veterinary medicine, health care and the preclinical sciences.

But finding the right citation is only one step. Connecting literature discovery with trusted, peer-reviewed full text can help medical libraries support evidence-based care, biomedical research and medical education. When users can move efficiently from discovering an article to examining the complete research, they can spend more time evaluating evidence and less time trying to locate it.

## Why Does Full-Text Access Matter in Biomedical Research?

Full-text access allows researchers to move beyond an article's citation and abstract to examine the methodology, complete findings, limitations and other details needed to evaluate the research.

Abstracts are valuable screening tools. They can help researchers determine whether an article is potentially relevant to their question. But an abstract cannot provide all the detail needed to critically assess a study.

This distinction matters across the communities medical libraries serve. A clinician investigating a treatment question, a research team conducting an evidence review and a medical student completing an assignment may have different objectives, but they share a fundamental need: access to the complete evidence.

For medical librarians, facilitating full-text access is an important part of supporting this process. Direct access can reduce the time users spend searching for articles through other channels and help keep their attention on evaluating the evidence.

## How Does Full Text Support Evidence-Based Practice, Research and Medical Education?

Medical libraries support a wide range of clinical, research and educational needs, all of which depend on access to complete biomedical evidence.

In hospitals and health systems, clinicians and research teams may need published evidence to support patient care, quality improvement initiatives or clinical research. Timely access can be particularly important when research is needed to inform clinical questions.

Medical colleges have many of the same research needs while also supporting faculty scholarship and educating future health care professionals. Students learning evidence-based medicine need opportunities to engage with complete research articles, critically appraise studies and understand how published evidence relates to clinical questions.

Medical libraries therefore support users with different roles, levels of expertise and research objectives. Comprehensive biomedical full-text resources can provide a common foundation for meeting those varied needs.

## Why Does Comprehensive Biomedical Journal Coverage Matter?

Full-text access is important, but access alone does not determine how well a resource supports biomedical research. The breadth, geographic diversity and timeliness of journal coverage also deserve consideration.

**International coverage supports broader research**

Biomedical research is global. Relevant evidence may come from researchers and journals around the world, making international journal coverage an important consideration for libraries supporting comprehensive literature searches.

Broad international coverage can help researchers identify evidence from a wider range of publications and perspectives relevant to their research questions.

**Ahead-of-Print content can improve access to emerging research**

Publication timing can also affect the research process. Ahead-of-Print articles make research available before assignment to a final journal issue, allowing researchers to discover emerging evidence earlier in the publication cycle.

When that content is also available in full text, researchers can examine the complete article rather than waiting for final issue publication.

**Embargoes can create a gap between discovery and access**

Embargoes present another consideration. When a journal's most recent articles are not yet available in full text, researchers may discover a highly relevant citation but still encounter an access barrier.

No-embargo journal coverage can help reduce the gap between discovering recent research and accessing the complete article.

These coverage considerations are particularly relevant for comprehensive literature reviews, where the objective is not simply to locate some relevant evidence but to identify the body of evidence appropriate to a research question.

## How Can Medical Libraries Connect PubMed Searches to Full Text?

Many biomedical researchers begin their literature searches in PubMed. When library linking tools are enabled, users can move directly from PubMed citations to available full-text articles in MEDLINE Ultimate, helping reduce barriers between literature discovery and access.

The quality of the underlying literature also matters. MEDLINE's journal-selection and indexing processes provide an established foundation for biomedical literature discovery. MEDLINE Ultimate builds on that foundation with full-text coverage of biomedical journals, helping libraries connect clinicians, researchers, faculty and students with evidence for clinical care, research and medical education.

## Build a Medical Library Around Today’s Research Needs

Access to full-text biomedical literature is an essential part of a research environment designed to support evolving clinical, research and educational needs.

*The Modern Medical Library Framework* explores the content, technology and capabilities medical libraries can consider as they work to support researchers and health care professionals throughout the research process.
